Expenses claim form: when claiming expenses, please include the following details: 1. Date of referral 2. Referring Organization. 3. Name of referrer and email/phone number contact 4. Client name (for GDPR reasons, it is better to give initials only); client gender; client age. 5. Cost claimed (note: invoices/vouchers will be required to validate your claim) and date claimed, 6. Brief description of items claimed e.g. 'full starter pack' or 'part starter pack'. No need to be too detailed at this stage! 7. Your first and last names. 8. Any comments you consider appropriate.
